1. Modified plpbt.bin with plpcfgbt.exe

2. Confirmed modifications with C:>plpcfgbt.exe plpbt.bin

Here they are:
Video mode     : 1024x768
Start mode     : menu
Countdown      : off
Countdown value: 15 seconds
Default boot   : off
Profile        : 1
Starfield      : off
Zoom animation : on
Font           : bios
Force USB 1.1  : off
Use USB MassDev: 1
Use INT19h     : off
Startup hotkey : no

3. Replaced the plpbt.bin file inside plpbt.img (using Ultra ISO)

4. Started VMware player with plpbt.img on the virtual floppy

Did not see the modifications:
a) from 640 x 480 -> 1024x768
b) change from default unwanted bootmanager font to the more readable font
c) removal of starfield animation

Does the bootmanager run default values if it thinks the IMG file is not right?

no

i never used ultraiso. try to delete plpbt.bin from the image and then copy the modified plpbt.bin to the image. and use a fresh plpbt.img from the zip file.

The replacement action taken with Ultra ISO of the BIN file was successful.

I extracted the replaced plpbt.bin out of the plpbt.img, and checked it with plpcfgbt.exe:  it represents the modified BIN file.

Ideally, I should probably extract the IMG file to a real (physically) 1.44 MB floppy, replace the BIN, and then create the IMG from the changed real floppy, except I don't have a real physical floppy.  I was thinking that a deletion and insertion (or replacement) of a file (BIN file) inside an IMG file would not really damage the IMG file.  I have no idea if Ultra ISO would affect or write over the parts of an ISO or IMG file outside of the space allocated for files (like boot sectors or allocation tables).

Did you try to build the ISO with the plpbt-createiso package?

ISO editors are not recommended for bootable ISO's.
